Community Support

Many people choose to remain at home during their illness but require the skills of our multi-professional team who are experts in palliative care. 

Our Hospice Outreach nurses and doctors work with you, your family and doctors and nurses in hospitals and the community to provide you with the best quality care possible.  They aim to enable you to remain in your own home for as long as you wish and will help you if you require care elsewhere.

If you are cared for by this team, you will have a named nurse allocated to you, but other members of the team may also be involved in your care. To contact the team for an informal discussion you can call 020 8320 5837.
