Greenwich Care Partnership
The Greenwich Care Partnership is a wholly integrated service which aims to deliver complete packages of care to people at the end of their lives. There is one contact telephone number for the service. The services work together to provide:
Palliative Care Coordination Centre
The team who are based at the Hospice will aim to provide a seamless service and:
- Act as a central point of communication for patients, their carers and professionals
- Organise and coordinate planned packages of care for people who are eligible for NHS Continuing care category 1 funded care
- Maintain the Greenwich End of Life Care Register
- Ensure handover of patient information to all involved health and social care professionals
Opening hours are:
- Monday to Friday 8 am to 6 pm
- Saturday, Sunday and Bank Holidays 8 am to 1 pm.

Rapid Response Service
Registered nurses will provide out of hours nursing care including:
- Triaging calls when the coordination centre is closed
- Providing planned and unplanned out of hours nursing visits
- Providing telephone advice and support for patients and their carers
- Providing palliative care advice to community care staff during the out of hours period
- Providing bereavement support immediately following death.
The service operates:
- Monday to Friday 4 pm to 8 am
- Saturday, Sunday and Bank Holidays
- 24 hours daily

Planned Night Care Service
This service is provided by:
- Senior Healthcare Assistants supported by the Rapid Response Service
The service operates:
- Between 10 pm and 7 am, seven days a week

Multi-visit personal care (hospice at home)
This service is provided by Healthcare Assistants trained in end of life care provision who deliver personal care and support to patients and their carers in their own homes.
Care includes:
- Personal care including help with washing, dressing, shaving and toileting
- Help with some domestic tasks such as preparation of drinks, basic meals and laundry
- Short visits to provide respite for the person’s carer

Who can be referred to the Greenwich Care Partnership?
Any person who is:
- Over the age of 18 with a diagnosis of a life limiting illness
- Registered with a Greenwich GP or be a Greenwich resident not registered with any GP
- Identified as being in the last year of their life.
Referrals can be made:
- To any element of this integrated service
- By any health or social care professional i.e. a key worker, discharge liaison nurse, specialist nurse, district nurse or GP
- Where a self referral is received, the person’s GP will be contacted for further information
