Hospice Outreach Team - Queen Elizabeth Hospital

The Hospital Support Team is a multi-professional team working in partnership with other healthcare professionals who work within Queen Elizabeth Hospital, Woolwich. 

We are able to help the patient with their symptoms such as pain and nausea and help them to make difficult decisions around their care.  We are also able to offer specialist advice, information, support and spiritual care and help them to adjust to their diagnosis and the changes they may experience in the course of their illness.

The team consists of Clinical Nurse Specialists, a Specialist Psychosocial Worker, Palliative Care Doctors, a Discharge Liaison Nurse, Counsellors and secretarial support.
