Hospice Outreach Teams - Community

For patients who wish to remain at home, a referral to our Hospice Outreach Team can be made by any healthcare professional. Care is provided by a team of experienced palliative care nurses, doctors and allied health professionals who are based at the Hospice. They will make home visits to assess the needs of the patient and their family or carer and give support and advice.
