Services
We offer a wide range of services and people may have their needs met by accessing an individual service or the full range of support. All of our services are provided free of charge.
Once referred to the Hospice, a member of the team will contact the patient or carer to arrange an assessment, following assessment if the member of the team and the patient/ carer agree that another service may be helpful, an onward referral will be made.
The team aims to work in as integrated way as possible, to avoid duplication and repeated assessments of patients, and we hold regular multidisciplinary meetings. If you would like to discuss a referral or a current patient's care with a member of the team, please do not hesitate to contact us.
Making a referral
Referrals are considered by the multi-disciplinary team within 24 hours. Someone will usually call you back with a decision once the referral has been discussed. Urgent referrals (i.e. requests for contact that day) are considered on an individual basis and the speed of outcome may be affected by a number of factors including patient caseload and need and availability of staff and beds.

It is sometimes helpful to discuss a referral on the telephone. Please do not hesitate to call on 020 8312 2244 to speak to a member of the Clinical Team.
For prospective inpatients or day care patients, it is possible for them and their families or carers to visit and take a look around. To ensure someone is available and sufficient time is given to answer any questions, an appointment should be made with the Ward Clerk or Nurse in Charge.
If a patient is already known to another palliative care team (or any other part of the Hospice service) it may be helpful to contact them in the first instance. They may be able to negotiate on your behalf and will have an up to date copy of the patient’s Hospice record.
