Support in Care Homes
The Hospice works closely with care homes in Greenwich and Bexley Boroughs to improve end of life care. Many care home staff feel isolated and do not necessarily have the support or skills they need to provide high quality care at the end of life. The Hospice has been playing its part in the implementation of the National End of Life Care Strategy and working with care homes is one of the many projects we have been focusing on.
We have built close relationships with care homes staff. Our input varies from training and education to hands-on support and advice. Training and advice helps staff to build their confidence when talking about the future with residents and their families. It also enables staff to continue to care for their residents until the end of their lives in the care home rather than transferring to hospital.
As part of this project, we support staff in care homes to implement end of life care tools which include the Liverpool Care Pathway and the Gold Standards Framework for Care Homes. By implementing these tools we hope older people and people with learning disabilities, living in care homes, will be enabled to live and die well in the place and manner of their choosing.
