The opportunity has arisen for an experienced palliative care nurse to lead the Palliative Care team at the Queen Elizabeth Hospital.
You will be responsible for coordinating a team of 4.6 wte Clinical Nurse Specialists, plus administrative and medical support within the Lewisham and Greenwich Trust based on the QE site in Woolwich. You will share your own palliative care skills and knowledge with your colleagues both in the team and beyond.
The service is moving to a 7 day 9-5 service once its complement of staff is filled and the Team Leader will facilitate that development.
The Team gives specialist palliative and end of life care and support to patients on the wards and to their families/carers, and gives expert advice to the hospital medical nursing and other staff caring for them.
You will set the standard for a high quality service, advise on all aspects of palliative care, help bring together the right care on discharge home, support patients’ preference for place of care and always advocate in the best interests of patient and family. You will demonstrate a high level of expertise and actively disseminate best practice through example, education and training, professional development and learning and participate in audit.
Based for most of the time at the Queen Elizabeth Hospital, your role will also require you to have on occasion a presence at Bostall Hill for management, education delivery and other meetings
You will be an ambassador for the Hospice within the Hospital and help connect Hospice with other services in each patient’s care pathway
